Surveillance of Australian Workplace Based Respiratory EventsThe SABRE Project in New South WalesWhat is involved? All participating doctors who see cases of occupational respiratory disease in their normal practice are requested to return SABRE notification forms every two months. Doctors indicate on this form (shown below) or the webform when they have no cases to notify. When notifying cases, participants now fill out the SABRE notification form (worksheet) and send it via standard mail or use the webform. To ensure that the reporting of the incidence of disease and the causal agents, occupations, industries are accurate, we will collect and use coded information about individuals to ensure that we can detect duplicate notifications. The webform, which is password protected, sends all data electronically to the SABRE Scheme and is a convenient method of notifying . Coding of cases To ensure we can detect duplicate notifications, take the last two initials of first name, second and third initials of surname followed by year and month of birth e.g. the code for John Jones with the date of birth 12/03/68 will be hnon6803. Place this code in the box labelled "Patient Code" on the worksheet.
